Group Benefits

Health, Dental, and Life Benefits

The United Church’s group insurance benefits are provided by Manulife Financial, and administered by Morneau Sobeco.

Administration and Claim Forms

For administration questions related to your health and dental, life insurance, or accidental death and dismemberment (AD&D) benefits (for example, change of address, new dependent), please log on to The United Church Pension and Benefits Centre ,* administered by Morneau Sobeco.

For Dental Claim forms and Health Claim forms, please log on to The United Church Pension and Benefits Centre * and go to the Forms section.

Is there a change in your life that could change your group insurance needs or choices? Consult Benefits for Active Members or Benefits for Pensioners to guide you in this matter, or log on to The United Church Pension and Benefits Centre .*

To log on to the Pension and Benefits website you will need:

  • User ID: The 5-digit Client ID number that appears on your drug card. (It also appears as the last 5 digits of your Employee/Payee ID on your pay stubs.)
  • Password: Your initial password is your birth month and day and the last 3 digits of your Social Insurance Number: mmdd123. The site will prompt you to choose a personal password when you log on.

Claim Reimbursements

For information about health and dental reimbursements, please log on to the Manulife website *, the church's benefits provider.
